| PROJECT SCOPE |
| The scope of the EHR project is to deploy, over the next 3-5 years, a functional enterprise EHR into the NEQCA Community |
| and Academic Provider Practices and the Academic Medical Center Ambulatory environment to improve the clinical |
| management of patients and improve the communication between community and academic medical center |
| providers. We expect this tool to improve the efficiency, accuracy and timeliness of data capture and clinical |
| documentation as well as improve provider practice workflow, reduce redundancy of information gathering. We also |
| expect this EHR to improve primary care-specialty provider communication and integration of the community |
| providers with their community hospitals and a major academic medical center. We expect this EHR to improve |
| patient communication and demonstrate, using data-driven quaility measures and clinical decision-making |
| tools, the network's ability to deliver high quality patient care. |
